The Consumable Blood Donation Program (CBDP) was established to provide a safe, accessible food supply for citizens who rely on consuming human blood to survive. We connect you with a provider and facility that can prescribe you with access to human blood based on your biological needs. We were founded as an open-enrollment option for those with non-violent anemiapepsia to survive off of human blood. If you need human blood, please put your faith in us to connect you to the best possible access to food. We have clinics in 13 states, and the sign up process is simple.
This is the request form for a consultation with a Program Advisor, who will then help you make an appointment with a doctor enrolled in CBDP (Consumable Blood Donation Program). Before you can be scheduled for an appointment, you must complete a pre-consultation confirmation questionnaire over the phone with one of our Program Advisors. These are registered nurses who work with our doctors to help coordinate the program and connect waitlisted recipients.
IMPORTANT! PLEASE READ: In order to qualify for access to a supply of human blood, you must provide proof of a doctor consultation confirming your biological need for consumable human blood. We require a FAX NUMBER to confirm this diagnosis.
We require a photograph ID with the submission of this form. Please include a copy of a government-issued photo ID.
