“B-but you weren’t. I- You were— You died.”
The harsh sobs continue, and the potentials of what could have been are still too strong for Izuku to fight off.
“And it's all my fault.”
That declaration stuns Katsuki; he jerks back, pulling away his touch as his eyes bulge with confusion. At his sudden withdrawal, Izuku crumples into his lap.
“No, please,” a mess of wildly curly hair buries into his abdomen, and hands falling to grip at his pant legs, “I’m sorry. I'm so sorry, Kacchan.”
Katsuki compels his brain to reboot, needing to understand and rectify where that thought process came from.
“Wh—”
But Izuku is already spiraling.
Whereas he had been fighting tooth and nail to get the dumb dork to talk ten minutes ago, now words are just fumbling out. This is the unstable coherence he had been hiding, choosing to instead shield behind a state of dissociation to avoid the overwhelming emotions.
“It's my fault he targeted you. He set out to specifically kill you because he knew what it would do to me. He knew that you were one of my only weaknesses. And I wasn't— I wasn't there because I— I was too late. I couldn't save you. Because I was too fucking late.”

Summary
Izuku was enjoying a rare and surprisingly quiet evening in the emergency department. They’d had the usual litany of drunks, elderly falls, and kids having shoved something up their nose that really didn’t belong there, but on the whole it had been a relatively easy night.
That all changed with the arrival of what seemed to be an entire squadron of police escorting several men in handcuffs through the ER doors.

Izuku is a doctor, and Katsuki is a police officer. Their paths cross in ER one night, and both can admit there is definitely attraction there. But then Katsuki ends up becoming one of Izuku's patients, and everything starts to change.

Summary
Katsuki has a nightmare/vision/whateverthefuck and wakes up tormented by it. Too many feelings bubble up at once and his chest starts to hurt from the crushing amounts of fear and confusion.
So, what's his first approach in response to all of this you ask?
Fucking the shit out of Izuku of course.

Summary
He’s afraid of mistaking Kacchan’s friendship for something it’s not. Sometimes Kacchan moves like he wants to take things a step further. He’ll stare at Izuku for long bouts of time when he thinks Izuku’s not paying attention.
His friends are convinced Kacchan is also into him and is too scared to make a move, but that doesn’t make any sense. Kacchan is always straightforward with what he thinks, if he really wanted to be with Izuku he would just say so. Right?
There’s only one reason left that Izuku can think of realistically that’s holding Kacchan back but he can’t explain it to anyone else. Because then that means facing something Izuku isn’t ready to acknowledge out loud.

Katsuki and Izuku are pretty much joint at the hip after the war, and it's pretty obvious to everyone they're in love, except for Kacchan and Izuku.
Or
Izuku deals with his insecurities about not having a quirk anymore and learns to accept that he's loved anyway.
