Basically quirks aren't really a thing in this AU.
There are a certain few people that have physical differences that are kept a secret and are locked up in hospital wards. Usually these patients are used for experimenting and are all around just not treated very well. Keigo is one of those patients and has a pretty good track record of escaping and getting caught.
But this time he actually got out of the building, but with no where to go he's contented to die out on the streets, at least he's not chained up anymore.
The poor bird, close to freezing and in nothing but plastic shoes and a hospital gown, is found by none other than Dabi/Touya Todoroki. Scarless but covered in tattoos and still the same stubborn ass he's always been, he's determined to take the birdie home.

Megumi tipped Gojo’s head up a little more and leaned down until his hot breath fanned across Gojo’s lips, “You’re what? Sa-to-ru?”
Another trickle of tears crept over his cheeks and Gojo gritted his teeth as he whispered, “Disgusting.”
“Good boy,” Toji said, and Gojo didn’t know who it was for. Him, or Megumi. He didn’t care. He had no time to care as Megumi lifted his head up and shoved his fingers deep into Gojo’s waiting mouth.
Gojo gets the shit railed out of him by Toji and Megumi.
