One is his scars, they'll always be with him, haunting him, even with the tigers regenerative abilities.
Two is his hallucinations, mostly his Headmaster, he recently has been leaving Atsushi alone though, however this is because of three.

Atsushi recently has been hallucinating more, but rather Shibusawa then the Headmaster. Some people are trying to figure out what is wrong, and some already have ideas.
