Summary
The doctor was speaking Federation Standard, which normally sounded to Garak like his own language, Kardasi (thanks to Universal Translators). Now, however, Garak was simply hearing the language that Julian was speaking. This wouldn’t necessarily be a problem – Garak did know Standard, having learned it from a young age as part of his training. Unfortunately, the doctor still thought he was plain and simple Garak the tailor, who would have no business knowing the Federation’s language.

The frustrating part was that Julian actually knew Kardasi, but he couldn’t tell Garak that. He’d learned the language (which was notoriously difficult for foreigners) in about two weeks after befriending Garak, and even read some of Garak’s book recommendations in the original Kardasi. But letting Garak know that would raise questions – questions that Julian wouldn’t be able to answer. So he had to pretend that he didn’t understand.
